Pure contract for the global operator brake consulted at every seam.

Fail-safe rule: if the engaged state cannot be read with confidence (a
corrupt / unreadable durable sentinel), `is_engaged()` MUST return
`True` — an ambiguous brake holds new work rather than letting it run
freely. `engage`/`disengage` are idempotent.
